This organisation is a small team, yet big on performance. They provide community based quality end of life hospice care in a close knit, caring community. Care is delivered in people’s homes and in aged residential care facilities across the diverse Tairawhiti region.
  
About the job
  • Holistic care coordination role based out of Gisborne but will involve one day per week travelling up the East Coast
  • Assessments, care planning, patient education, identifying equipment needs.
  • Supporting patients and their whanau. 
  • 4 days per week Monday to Friday (will consider full time) 
  • On call component. 
  • The busiest time is when you are working with patients trying to get symptoms and pain under control, or if they are actively dying.
  • Every day starts in the office but then you can either visit acute patients with the Doctor, see your own patients in the community, or stay back in the office taking phone calls and doing paperwork.
About you
  • A passion for providing quality end of life care is essential
  • Hospice, aged care, or specialist palliative care experience is preferred
  • Registered Nurse with a current NZ practicing certificate and right to work in New Zealand
  • A proactive, flexible, team player with a calm, confident and even temperament
  • Current and Full New Zealand Drivers license
  • Connections to / relationships with Ngāti Porou Iwi essential
