Holistic Hospice Community Nursing role - care coordination, assessments, pain and symptom management, time to work closely with patients and whānau

• This is an holistic care coordination role. Be responsible for assessments, care planning, patient education, identifying equipment needs, as well as supporting patients and their whanau. 
• The busiest time is when you are working with patients trying to get symptoms and pain under control, or if they are actively dying.
• Every day starts in the office but then you can either visit acute patients with the Doctor, see your own patients in the community, or stay back in the office taking phone calls and doing paperwork.
• This position is Monday to Friday (will consider part or full time) 
• There is an on call component. This is flexible during the week and one in five weekends. 

• A passion for providing quality end of life care is essential
• Hospice, aged care, community or specialist palliative care experience from within New Zealand is preferred
• Registered Nurse with a current NZ practicing certificate and right to work in New Zealand
• A proactive, flexible, team player with a calm, confident and even temperament
• Current and Full New Zealand Drivers license
